Cellular Telephone Agreement Accordion Closed
An ITS employee who is eligible to receive a cellular telephone purchase reimbursement or service allowance. To initiate a cellular telephone purchase reimbursement or monthly service allowance, employees must seek approval of their supervisor, and complete the Cellular Telephone Purchase and Service Allowance Agreement.

Flex/Telecommute Request Accordion Closed
If you are working from home for any reason related to COVID-19, you will need to complete the Flex/Telecommute Request form.
Important things to note:
- This process will assist ITS in tracking who in our department is working remotely.
- Even if you have an email agreement in place with your supervisor, please complete the new online form.
- If you were on an approved paper Flex/Telecommute request prior to COVID-19, you don’t need to do anything at this time.
- While Flex/Telecommute requests are normally not designed as a replacement for family care, during COVID-19, supervisors should be flexible in allowing employees to work from home, including to manage childcare/family care changes, if the employee is able to perform their job duties from home and has the equipment, access and data security to do so.
Home Use of University Equipment Accordion Closed
Working from home, related to COVID-19, will be an agreement between you and your supervisor. Arrangements should be documented via email, summarizing expectations. The Home Use of University Equipment form should be completed prior to taking NAU equipment home.
