Report a concern or incident
Anyone may report a concern about an NAU student. Listed below are a number of different reports for different topics. If you would like to first consult with a staff member before submitting a report, please call the Office of Student Rights & Responsibilities at 928-523-5181.
Please note that review of these forms will not take place outside of normal business hours. If there is an immediate risk to health or safety, please call 911.
Conduct and CARE Reports
Student Code of Conduct Report Accordion Open
Use the Student Code of Conduct Report to report violations of the Student Code of Conduct. Violations include, but are not limited to: Unlawful alcohol and/or drug use, physical assault, hazing, and theft. Members of the Flagstaff community may also report concerns about general civility and the behavior of students who reside off-campus.
CARE Referral Accordion Closed
Faculty, staff, and students are encouraged to submit a CARE Referral to report concerning behavior that may be impacting a student’s academic success. Concerns about absences, hospitalizations, death, mental health, etc. may be reported here. The CARE Team will reach out to this student to assess what support or resources they may want or need. Faculty who wish to share academic concerns can do so via F2S comments.

Sexual Misconduct Report Accordion Open
Use the Sexual Misconduct Report to report an incident of sexual misconduct. If you are a student, you may do so anonymously or provide your information. The choice is yours. If you are a NAU faculty or staff member, you are required to provide all the details shared with you. Exceptions include legally protected personnel such as counselors, clergy, attorneys and doctors, who are acting within that role. If you have any questions regarding your obligation to report, please contact the Office of Student Rights & Responsibilities at 928-523-5181 or the Office for the Resolution of Sexual Misconduct at 928-523-5315. Sexual Misconduct includes gender-based discrimination, sexual harassment, dating violence, domestic violence, sexual assault, or stalking behaviors. Campus Security Authority Report Accordion Closed
The Clery Act requires the university to collect crime reports from a variety of individuals and organizations that Clery considers to be Campus Security Authorities or CSAs. Please use the Campus Security Authority report to submit a crime statistic report to the NAU Police Department. More information can be found on the NAUPD website.
Anonymous Police Report Accordion Closed
Use the Anonymous Police Report to alert police of criminal activity. DO NOT submit a report for any crimes in progress (e.g. odor of marijuana, active arguments, or other situations requiring immediate police response). Call 911 for an immediate police response. Please fill out all fields as completely as possible. Remember that false reporting is a crime and will be prosecuted.
Academic Reports
Disruptive Behavior in an Instructional Setting Accordion Open
Submit a Disruptive Behavior report to document an incident that an instructor believes may constitute prohibited Disruptive Behavior as defined by NAU policy.
Academic Integrity Accordion Closed
NAU expects every student to firmly adhere to a strong ethical code of academic integrity in all their scholarly pursuits. The Academic Integrity form should be used to report violations of the Academic Integrity Policy.
