Forms information
Unless otherwise noted, all forms are compatible with both Microsoft Word and Adobe Acrobat format. If you do not have Microsoft Word, then visit Adobe and download Acrobat Reader for free.
Please note that not all forms are designed to be completed from the browser screen, but rather, completed after they are printed.
All forms must be physically signed and/or send from your NAU email address to the email address on the form. The Registrar’s Office will not be able to process an electronically signed form.
Changes to Student Information Forms Accordion Closed
Change of Information – Date of Birth and/or Legal Sex PDF or fill out the Online Change of Information form.
To change your social security number, please use this W-9S form.
Change of Major, Minor, or Certificate form
Change of Name PDF or fill out the Online Change of Name form.
Petition to Change Residency Status Forms – Arizona residency classification has moved. For more information, please visit the Student Service Center website.
Enrollment & petition forms Accordion Closed
Enrollment Add/Drop Request. For immediate processing, log on to LOUIE and add/drop classes in self-service.
Override Authorization. Complete for class capacity, time conflict, requisites for course, department or instructor consent.
Petition to Add After the Deadline.
Petition to Change to A/Pass-Fail After the Deadline.
Petition for Full-Time Below 12 Units.
Petition to Withdraw from a Class After the Deadline.
Repeat Enrollment for No Credit Form.
Request for A/Pass-Fail Grading Option.
Request to Waive Late Registration or Petition Fees PDF or fill out the Online Fee Waiver form. To be completed by the department owning the NAU Error.
Unit Load Override Approval – Log into LOUIE and click on Registrar’s Office Forms; click on Add Unit Load Override; select your academic career; select a term; select class(es); and submit. You will receive an email confirming your form was submitted.
