You can choose from a variety of medical, dental and vision plans with competitive employee premiums. Each plan has different coverage levels and out-of-pocket expenses.

What to consider

There are three important things to consider when selecting your medical, dental and vision coverage.

What costs do you expect in the coming year?

        Your Cost =   Employee Premiums + Out-of-Pocket Costs (deductibles, copays & coinsurance)

Are your providers in the plan’s network?

Visit the Vendor Contact webpage for the provider’s website address and customer service phone number.

What does the plan cover?

Visit the Benefit Plan Documents and Policies web page for plan details including the Summary of Benefits and Coverage for each medical plan.


Medical Plans

You can choose from 4 medical plans that all include:

  • Nationwide network
  • Prescription coverage
  • Preventive care at no cost
  • In and out of network coverage

Highest premium but lowest out-of-pocket maximum.

  • BCBSAZ
  • Copay before deductible for prescriptions
  • No deductible or copay for Campus Health Services or mental health office visits
  • Copay or coinsurance after deductible for most other services
  • Eligible for a Health Care FSA but not an HSA.

NAU High Deductible Plan Accordion Closed

Lowest premium but high deductible

  • BCBSAZ
  • Copay after deductible for prescriptions
  • Coinsurance after deductible for most other services
  • No cost at Campus Health after deductible
  • NAU contributes to Health Savings Account (HSA) with Health Equity – Learn how an HSA works.
  • Eligible for a Limited Purpose FSA

State of AZ Triple Choice Plan – TCP Accordion Closed

Low premium, but highest out-of-pocket maximum.

  • Choice of UHC or BCBSAZ
  • 3 network tiers
    • Tier 1 – Preferred – lowest deductible
    • Tier 2 – In-Network
    • Tier 3 – Out-of-Network
  • Copay before deductible for prescriptions.
  • No cost at Campus Health after deductible
  • Copay or coinsurance after deductible for most other services
  • Eligible for a Health FSA but not an HSA.

State of AZ High Deductible Plan Accordion Closed

Very low premium but high deductible

  • Choice of UHC or BCBSAZ
  • Copay for prescriptions after deductible.
  • Coinsurance after deductible for most other services
  • NAU contributes to Health Savings Account (HSA) with Optum – Learn how an HSA works.
  • Eligible for a Limited Purpose FSA

Dental Plans

You can choose between two dental plans: a PPO and a DHMO. The plans are very different. They have different out-of-pocket expenses, employee premiums and networks.

State of AZ PPO – Delta Dental Premier Accordion Open

Higher premium but a large network

  • In & out-of-network coverage
  • Your costs are lower when you see providers in the network
  • For out-of-network services, you are responsible for balance billing amount
  • No cost for preventive care
  • Deductible and coinsurance for basic and major services
  • $2,000 Maximum annual benefit
  • $1,000 Lifetime orthodontia benefit

State of AZ DHMO – United Health Care – Solstice Accordion Closed

A low premium but a limited network – few providers in Northern AZ.

  • In-network coverage only!
  • You are responsible for the full cost of out-of-network services.
  • No cost for preventive care
  • Schedule of Benefits for non-preventive services
  • No annual benefit maximum

Not available in the following states: AK, AL, AR, DE, HI, IA, ID, LA, ME, MS, MT, ND, NE, NH, OK, RI, SD, VT, WV, WY, GU, USVI, and PR

Vision Plan

NAU offers the Avesis Advantage Plan for vision coverage.

  • In & out-of-network coverage
  • Lowest cost with in-network providers
  • Limited reimbursement for out-of-network services
  • Yearly coverage for a routine eye exam, glasses or contact lenses.
