Insurance and payment

In response to the rising need, healthcare providers are moving into a new area to provide telemedicine services at Campus Health Services. Telemedicine lowers risk of exposure while maintaining accessibility to your healthcare provider. Telemedicine visits are billable services but all insurance plans may not cover telemedicine, even in these changing times.

We advise all patients who wish to utilize CHS telemedicine for medical or psychiatric care to reach out to their insurance company. Ask whether Telemedicine services are a covered benefit under your plan. Telemedicine services will be billed at the same cost as an in-office, In person visit.

Our services are available for all currently-enrolled Northern Arizona University students who have paid the mandatory semester Health and Recreation Fee. Insurance is not required to receive our services; however, it is suggested.  Services are also available for Northern Arizona University staff and faculty and their adult dependents.

Plans

You can choose from these four plan options in order to get treatment from Medical Services:

  • Fee for Service: You pay for individual services and are charged $58 per office visit along with any other labs or procedures you have done.
  • LouieCare: You pay $135 per semester and most services provided by Medical Services are covered with a $10 co-pay per service.
    • The LouieCare must be paid in full to be in effect. Any services received before the plan is paid in full will not be covered at the discounted rate.
  • Student Health Insurance Plan (SHIP): This group health insurance product is offered by all three state universities in Arizona and provides group coverage to students who are uninsured, need a less expensive alternative, or need an Arizona provider network. Primary care is covered at Medical Services with a $15 co-pay for each office visit and $10 for any lab.
  • Your own contracted insurance: We are currently contracted with Blue Cross Blue Shield, Aetna, Cigna, United Healthcare, and TriCare. We are currently not contracted with any state Medicaid plans, including AHCCCS plans.

Additional information:

  • CHS must receive insurance information (copy of front and back of card) in order to bill any contracted insurance.  You may upload your card images prior to your appointment through the patient portal:  www.campushealth.nau.edu.  The card may also be uploaded at your appointment.  If the insurance information is received after the date of service, any incurred charges may become the patient’s responsibility.
  • An Insurance Billing Form must be completed and presented with your insurance card in order to bill for services.
  • Any copays, deductibles, or denied charges may be posted to the LOUIE account. You should verify your benefits through your insurance company and if you are from outside the Arizona network or have an HMO, you should check with your plan to verify coverage.
  • CHS uses a 3rd party laboratory service (Sonora Quest) for certain lab services and you may receive an additional bill from Sonora Quest.
For additional information contact our insurance office at 928-523-6343.

How does insurance work? Accordion Closed

Let’s say, for example, you are sick and make an appointment at NAU Campus Health Services for evaluation and treatment. You will provide your insurance card and NAU ID to registration staff. Usually, there will be a co-pay amount required by your insurance.  After registering, the NAU Campus Health Services provider will examine you and discuss with you what tests or treatment you may need.

What happens after my visit? Accordion Closed

NAU Campus Health Services will submit your charges and information to your insurance company. Your insurance company will process the NAU Campus Health Services claim for payment according to your plan benefits and sends the payment to NAU Campus Health Services. Your insurance company will also send you a notice (by mail or electronically) that the claim has been processed. This notice is called an Explanation of Benefits or EOB.

What if NAU Campus Health Services is not contracted with my insurance company? Accordion Closed

NAU Campus Health Services does not bill non-contracted insurance companies.  In these cases, you are expected to pay for your charges at the time of your visit.  If you do not see your insurance company on our contracted list, or if you are unsure, contact your insurance company and ask if your plan covers services rendered at NAU Campus Health Services. If we are not contracted, you may want to consider purchasing LouieCare.

If your insurance company does not pay for the lab test in full, you will receive a separate bill from Sonora Quest.

What are some of the reasons my insurance won’t pay? Accordion Closed

  • You received services from an out-of-network provider.
  • You did not call your insurance company for prior approval before receiving treatment, if required.
  • The service you received is not covered under your plan, for example MMR vaccination, STI tests, etc.
  • Your insurance company needs verification of your student status
  • You have an HMO plan and it only covers you when you see the doctors or healthcare facilities within their network. We are not contracted with any HMO plans.
  • Your coverage has expired.
  • You switched insurance plans but forgot to notify NAU Campus Health Services or upload your new insurance card through your patient portal.

What are my payment options? Accordion Closed

If your charges were sent to your Louie Account, simply logon and pay your charges online. To pay charges in person, go to the Campus Health Services Administration Office in the Health and Learning Center, Building 25, on the second floor.

Who do I contact with questions about my insurance claim or my bill? Accordion Closed

First contact your insurance company via telephone or the plan website. You may contact the NAU Campus Health Services Billing Office at 928-523-2131 or at campushealth@nau.edu. It is important to leave a message with your name, NAU ID number, contact information and question. We will contact you within 24 hours.

What about my confidentiality? Accordion Closed

If you are 18 or older, we cannot discuss your charges with anyone except you, including your parents, without your permission. By signing the consent to treat, you authorize us to release information to your insurance company to process your claim. Recognize, however, if the billing address for your insurance plan is your parent’s/spouse’s/significant other’s home, an EOB from the insurance company (see insurance How Insurance Works) will be sent to that address. If you want us to discuss your health charges with your parent or spouse, you can fill out the Release of Medical Healthcare Information form in your Patient Portal.  This will allow others to have access to your information (scheduling treatment, discuss treatment, handle finances concerning health care).
