How to Complete the Billing Form
- Launch your MyHealth secure portal at https://nauportal.pointnclick.com/
- Select “NAU Student/Employee”
- Use your NAU username and password to authenticate
- On the left side of the screen, select “Required Forms”
- For the item titled “Billing Form” select “Update“
- Enter all required information
- Patient Name
- Patient Date of Birth
- Contact Number
- Choose ‘Yes’ or ‘No for the question “Do you have insurance that you would like CHS to Bill?”
- Choose your payment information from the choices listed
- Insurance Information Section:
- If your insurance is through a parent, or another individual, please list their name and information in the section. If you are the policy holder, please use your information.
- If you don not have insurance, please enter “none” into each field.
- Check the box at the bottom for your e-signature and enter the date in MM/DD/YYYY format.
- Click “Submit Final“
