What are my options to pay for my visit? Accordion Closed
You can choose from these four options to pay for your visit at Campus Health Services:
- Fee for Service: As a student, if you have paid the Health and Wellness Fee, you pay for individual services and are charged a fee for your office visit. If you have not paid the Health and Wellness Fee, Office Visits are charged at a higher fee schedule. Potential out-of-pocket costs in addition to the office visit fee include laboratory, injections, procedures, and supplies.
- LouieCare: You can pay $175 per Fall, Spring or Summer semester, $350 per academic year, or $460 per full year. Most medical services provided by Medical Services are covered with a $15 co-pay per service. In-House laboratory tests are $15.
- Student Health Insurance Plan (SHIP): This group health insurance product is offered by all three state universities in Arizona and provides group coverage to students who are uninsured, need a less expensive alternative, or need an Arizona provider network. Primary care is covered at Medical Services with a $15 co-pay for each office visit and $10 for in-house laboratory tests.
- Your own Contracted Insurance Plan: We are currently contracted with Blue Cross Blue Shield, Aetna, Cigna, United Healthcare, and TriCare. We are currently not contracted with any Kaiser, and state Medicaid plans, including AHCCCS plans. To verify our participation, please call the number on the back of your insurance card and ask if they are contracted with NAU Campus Health Services. Our Tax ID is 74-2579628.

If you are 18 or older, we cannot discuss your charges with anyone except you, including your parents, without your permission. By signing the consent to treat, you authorize us to release information to your insurance company to process your claim. Recognize, however, if the billing address for your insurance plan is your parent’s/spouse’s/significant other’s home, an EOB from the insurance company (see insurance How Insurance Works) will be sent to that address. If you want us to discuss your health charges with your parent or spouse, you can fill out the Release of Medical Healthcare Information form in your Patient Portal. This will allow others to have access to your information (scheduling treatment, discuss treatment, handle finances concerning health care).
