Life Insurance
NAU offers employees three types of life insurance coverage:
- Basic Life
- Supplemental and
- Dependent
Basic Life Insurance
You are provided $15,000 of Basic Life Insurance and $15,000 of AD&D insurance at no cost to you through the State of AZ plan with the Hartford.
Supplemental and Dependent Life
You can purchase coverage through both NAU’s plan through the Hartford the State of AZ’s plan.
NAU – The Hartford Accordion Open
Supplemental
- When eligible, you may can purchase 1,2, or 3 times salary. The maximum benefit is the lesser of three times salary or $300,000.
- During Open Enrollment, you may purchase additional 1 times salary.
- Coverage amount reduced 40% age 70+
Dependent
- You may purchase the following amounts: $5,000 or $10,000
- Covers your spouse & eligible dependents.
- You are the beneficiary.
State of AZ – Securian Accordion Closed
Supplemental
- When eligible, you may purchase coverage in increments of $5,000. Maximum benefit is the lesser of three times salary or $500,000.
- During Open Enrollment, you may purchase an additional $20,000 of coverage.
Dependent
- You may purchase the following amounts: $2,000, $4,000, $6,000, $10,000, $12,000, $15,000 or $50,000.
- Covers your spouse & all eligible dependents.
- You are the beneficiary.
- To purchase $50,000, you must enroll in $35,000+ of supplemental coverage.
Dual enrollment of Supplemental and Dependent life is not allowed. You, your spouse and dependent child can only be covered once under each option. For example, if you and your spouse are both eligible for State of AZ benefits, then you cannot be enrolled as employee under your plan and as a dependent under your spouse’s plan.
Beneficiaries
In the event of your death, your basic life and supplemental individual life insurance benefit will be paid to your designated beneficiary. If you choose more than one beneficiary, you can define the amount paid or a percent paid to each beneficiary.
- A primary beneficiary receives 100% of the designated amount/percent.
- A contingent beneficiary receives a benefit only if all of the primary beneficiaries are deceased.
Portability and Conversion
Within 31 days after your life insurance ends, you may choose to continue your supplemental life insurance coverage (called “porting”) or convert it to a whole life policy. Term life insurance provides a death benefit only, while whole life insurance also builds cash value.
