Unum short term disability claims

While on a Leave of Absence you may be eligible for income replacement through Unum. To file a short term disability claim you will need to complete a Unum Claim form.

After you and your physician complete each of your respective sections on the claim form, it will need to be remitted to Human Resources, who will complete the employer’s section and then remit it to UNUM on your behalf.

If you are hospitalized or have an out-patient procedure, then please have your physician and/or hospital staff complete a Unum Hospital form. It can be faxed directly to Unum or it can be remitted to Human Resources and we can send it to Unum on your behalf, whichever is more convenient for you.
