Signature Certifications and Mailing Instructions
Completion and Signature
Do not accept any Remote Employee Hiring Packets that are incomplete (e.g. missing signature, missing elections per applicable sections of the hiring packet or via facsimiles). When the employee provides their signature at the end of the hiring packet, they are certifying that all of the information they provided per sections are true and correct. Their signature also certifies that they have read, they understand and they have made applicable elections on each section of the hiring packet, including but not limited to the following:
- Personal Information, Mailing Address, Emergency Contact
- Level of Education
- Selective Service certification
- Veteran Status classification
- Receipt of Affordable Care Act Consent
- Loyalty Oath affirmation
- Conviction Disclosure
Mailing the Hiring Packet
To protect the employee’s personal sensitive information, the employee may elect to mail the hiring packet along with their completed Form I-9 with supporting identity and employment verification documents through a secure delivery, such as FedEx, to:
Northern Arizona University
Human Resources/Hiring Packet
PO BOX 4113
Flagstaff, AZ 86011
It is not recommend that the employee sends their hiring packet electronically because the email path may not be secured, but if it is more convenient for them, they can scan their completed and signed hiring packet along with your completed Form I-9 with supporting identity and employment verification documents and email it to HR.Contact@nau.edu with a subject line, Human Resources/Hiring Packet.
Upon receipt, Human Resources will send the employee an email confirmation, to the email address that they provide on the hiring packet, indicating that their hiring packet and completed Form I-9 has been received. If you need any assistance, please contact Human Resources at 928-523-2223 or HR.Contact@nau.edu.
