Report of Injury (ROI)
The online ROI should be used to report an employee’s injury or illness that occurred during work hours.
Using the ROI
The online ROI is available in Employee Self – Service through LOUIE – HR.
- Sign on to LOUIE – HR with your NAU credentials.
- Select Manager/Department Dashboard tile.
- Select the Manage eForms tile
- Select Report of Injury Home Page from the menu
- Choose Create a Report of Injury

Third Party Report of Injury
You can also use the online ROI to report a non-employee injury or an employee injury that occurs outside of work hours. Learn more about the process to report a Third Party Report of Injury
