Important information about your QLE
- ID Cards: New ID cards may take up to 14 business days to receive once the appropriate provider(s) have processed the change(s).
- Premium rates: The benefits team will review your changes to determine if there are additional premium amounts due or if a premium refund is owed to you.
- Other coverage: If you and/or dependents (spouse and/or child) also have health care coverage through another State of Arizona employer contact the Benefits team.
Updating other benefit and personal information
As a result of your QLE, you may want to update other benefit and personal information.
Life insurance beneficiaries Accordion Closed
How to add/delete your life insurance beneficiaries in Employee Self-Service through LOUIE – HR .
- Select Benefits tile.
- Select Benefits Summary from the menu.
- Click “NAU Basic Life” or the Life Insurance plan you wish to update.
- Click “Edit” at the bottom of the page.
- Enter or change a primary and/or contingent allocation. Please note that percentages must add up to 100%. Click Save
- If you would like to completely remove a beneficiary please contact HR at 928.523.2223 or nauhrbenefits@nau.edu
Add or drop a dependent during a QLE Accordion Closed
How to add or drop your dependent information in Employee Self-Service through LOUIE – HR.
- Select Self Service > Benefits > Benefit Enrollment
- Click on the benefits you wish to update by selecting “Edit”
- Click “add/enroll dependent” by selecting the dependents name at the bottom of the page
- To add, enter the dependent’s information
- To drop a dependent, uncheck the dependents name
- Click “update and continue”
- The coverage level updates after you selected/entered the dependent information AND click update and continue
- Please note you will need to go through each benefit by clicking “Edit” to add/drop your dependents

How to update your emergency contact information in Employee Self-Service through LOUIE – HR.
- Select Self Service > Campus Personal Information > Address
- Select the emergency contact tab
- To add a new contact click “Add An Emergency Contact”
- To update the information for an existing contact click “contact name”

Please note the “home address” field is the address our benefit vendors use to send insurance cards and any other important correspondence.
How to update your address through Louie– Employee Self-Service.
- Select Self Service > Campus Personal Information > Address
- Select the address tab
- To add a new type of address click “Add A New Address”
- To update and existing address type click “edit” under the address type

- Click on “Self Service”
- Click on “Payroll and Compensation”
- Select the tax form you wish to update

If you, a dependent, or a plan beneficiary has a name change, you should apply for a new Social Security card with Social Security Administration. Once you receive a new Social Security card bearing your new name you should:
- Complete the Employee Primary Name Change Form
- Enter your seven-digit employee identification
- Check the appropriate box for the name change and enter the date of the event
- Provide all of the essential information in the space provided
- Sign, date, and print name legibly
- Print and return the form along with the original Social Security card and photo identification to Human Resources for processing
