Site authorization

All research applications must be submitted to the IRB through IRBNet, an externally-hosted software product. All IRB forms must be obtained from the IRBNet Forms and Templates library.

Request for investigators who are not affiliated with Northern Arizona University to access University personnel, students, or resources to conduct human research.

Site authorization submission

All site authorizations require the following:

  1. Complete and upload the Site authorization form.
  2. Approval letter from the reviewing Institution.
  3. Original approved application from the reviewing Institution.
  4. Original approved informed consent from the reviewing Institution.
  5. Share your project with the NAU IRB Office, granting “Full” access.
  6. The PI must sign the package.. Student projects must be signed by the faculty advisor as well as by the student PI.
  7. Submit the package by clicking the “Submit” button in IRBNet. Select “Other” from the “Submission Type” dropdown options.
