GEAR UP Family – thank you for completing this form to inform us that you received the letter with information on your child’s plan after high school! By completing this form, your student will be eligible to receive a free Hydroflask water bottle. If you have multiple students enrolled at a GEAR UP high school, please complete one of these forms per child. One Hydroflask will be awarded to the first 25 families to complete the form per GEAR UP high school. Please provide your most recent contact information so that GEAR UP can contact you to inform you if your child received the water bottle.
