Self-identification process
Welcome!
The first part of the Disability Resources eligibility process is to self-identify as a student with a disability and request accommodations.
In order to evaluate accommodation and service requests, Disability Resources will need information about how your disability is likely to impact you here at Northern Arizona University.
Any information or documentation students can readily share during or prior to their first meeting is helpful and can save the need for follow-up meetings with Disability Resources. Disability Resources recognizes that barriers exist and can create hardship for students with regard to having and presenting third party disability documentation. Please don’t delay submitting the self-identification form out of concern for not having appropriate paperwork. If you have information or documentation, you will be provided an opportunity to upload it with your self-identification form.
You can also submit information or documentation to Disability Resources by:
- Bringing it to our office in Room 2050 at the Health and Learning Center, Building 25
- Emailing it to DR@nau.edu
- Faxing it to 928-523-8747
- Mailing it to: Disability Resources, P.O. Box 5633, Flagstaff AZ 86011-5633

It may take up to four weeks to review and process your information or documentation. Freshman and new students are strongly encouraged to request accommodations from Disability Resources as early as possible prior to attending Northern Arizona University. Current students may request accommodations at any time but are encouraged to make their requests for accommodations as soon as they become aware of a need for one.

Online Self-Identification Form Tutorial
To begin the self-identification process, navigate to the self-identification form. You will be required to log-in using your NAU User-ID and password. In the personal information section, you will be asked to enter your student number. This is a nine-digit number that typically begins with two zeros. When entering your student number in the Self-Identification Form, do not enter the leading zeros; enter the seven numbers after the two leading zeros.
