I consent to the following:
I hereby agree to participate in focus group research administered by Northern Arizona University (NAU). I also grant permission for NAU and the Arizona Board of Regents (ABOR) to create and use video and audio recordings of my face and voice as part of my participation in the focus group research. I understand that the recordings will be used to improve websites, web applications, and other products and services at NAU, and I assign all rights for use to NAU.
I understand that I will not receive any compensation now or in the future for my participation nor for use of the recordings.
I am at least 18 years old or my parent/guardian is completing this form on my behalf.
