New IRB project

All research applications must be submitted to the IRB through IRBNet, an externally-hosted software product. All IRB forms must be obtained from the IRBNet forms and templates library.

Instructions for student applications

  • Faculty members must serve as PI on undergraduate research applications.

All new projects require the following:

  1. Complete the NAU research application within IRBNet.
  2. Complete and upload the Project narrative.
  3. Complete and upload the List of project personnel. Tip: Ensure that all personnel on your project have completed human research Collaborative Institutional Training Initiative (CITI) training, be sure to include yourself and your CITI training date.
  4. Upload a CV or biosketch for the PI, all CO-PIs, Co-Investigators, and the faculty advisor.
  5. Share your project with the NAU IRB Office, granting “Full” access.
  6. The PI must sign the package.. Student projects must be signed by the faculty advisor as well as by the student PI.
  7. Submit the package by clicking the “Submit” button in IRBNet. Select “New Project” from the “Submission Type” dropdown options.

Additional requirements:

Based on the type of research you are doing you may also need to submit the following:

  • Informed consent/permission/assent form(s) – including study-specific release of information documents, DHHS approved sample consent forms. If consent will not be documented in writing, a script of information to be provided orally to subjects.
  • Data collection tools – surveys, questionnaires, data elements for records reviews, etc.
  • Participant materials – written materials to be provided to or meant to be seen or heard by subjects (e.g. instructional materials, study newsletters, physician to participant letter, wallet cards, incentive items, holiday/birthday cards, certificates, etc.)
  • Recruitment materials – telephone and verbal scripts, flyers, classroom announcements, brochures, websites, email texts, radio/television spots, newspaper advertisements, press releases, etc.
  • Grant application(s) and notice(s) of grant award – cover-to-cover copy of grant and award letter, regardless of home institution or funding agency.
  • Site authorizations for research purposes and/or access to administrative records/samples.
