Reporting options
Administrative reports Accordion Closed
An individual may choose to report an incident of sex/gender-based discrimination, including, but not limited to sexual harassment, sexual assault, dating violence, domestic violence, or stalking, using the Sexual Misconduct Report Form. The university will process reports through the Sexual Misconduct Policy using one of several procedures. These processes are administrative, internal, university processes. If desired, one may pursue concurrently an external, criminal investigation.
Criminal incident reports Accordion Closed
Individuals may choose to report as much or as little information as they wish when they file a report with a law enforcement agency and decide how involved they wish to be in prosecution. Depending on the location of the incident, a report may be made through:

The police department will send a reporting officer to the individual’s location and ensure that they are in a safe place before asking for basic information about the incident. Often, the reporting officer will then have dispatch contact Victim Witness Services, who will help the individual understand their options, including a forensic exam or “rape kit” through Northern Arizona Care and Services After Assault (NACASA).
Confidential reporting Accordion Closed
If an individual talks to a counselor at Employee Assistance and Wellness, 928-523-1552 (employees) or Counseling Services, 928-523-2131 (students), the information given will remain confidential in accordance with the law. The counseling relationship provides a safe space for impacted parties to talk about their situation and the resources available to them.
Anonymous reporting
In an anonymous report, no names are required. No information is included that might identify the individual; an individual can choose how much information is shared.
Individuals may use the Sexual Misconduct Report Form to report one or more incidents of sexual misconduct to the university. To report an incident anonymously, fill in the “Remain Anonymous” box in the report.
The Northern Arizona University Police Department (NAUPD) Anonymous Report Form also can be used to make an anonymous report of sexual assault or other sex/gender-based violence.
If a member of the NAU community is in danger, NAUPD or the Office of Title IX Compliance and Services (TIXCS) will notify the necessary departments with the information. Otherwise, the anonymous report is used to track possible patterns or areas that could pose a danger to the campus community.

Victim Witness Services
If desired, survivors of sex/gender-based violence may contact Victim Witness Services to discuss their reporting options and what to expect in various processes. Victim Witness Services can be reached at 928-856-7676.
Forensic exam
A forensic exam can be done up to 120 hours (around five days) after an incident of sexual assault or domestic violence. A police report is NOT necessary to obtain a forensic exam. Forensic exams are voluntary and can take up to six hours. Generally, the person receiving a forensic exam may bring another person for additional support. During a forensic exam, a forensic nurse examiner will ask questions about the incident and conduct a comprehensive physical exam. The person receiving the forensic exam can stop the exam at any time. To schedule a forensic exam, contact Northern Arizona Care and Services After Assault (NACASA) at 928-856-7676.
