This training needs to be updated if older than one year from admission to the College of Nursing. An NAU login to Canvas is required.

If you are new to NAU, access to the Academic Integrity training requires enrollment in your nursing courses. This requirement will be available in Canvas after the start of the semester. Please upload to Typhon no later than the 2nd week of the semester.

Please check back after you have been enrolled in your nursing courses. Contact University Admissions to ensure you have paid your application fee and have been fully admitted to the university.

Students will use Typhon, an online student portfolio to maintain clinical documentation and performance sheets throughout the program.

Mandatory Document Checklist

Please complete the Admissions Contract form to accept or decline admission to the Sanghi College of Nursing program.

The College of Nursing will enroll students in nursing courses. Please complete the following:

• Drop all enrolled courses for the upcoming term.
• Complete your TO DO list on Louie and clear all holds.
• If a transfer student to NAU, contact Admissions to ensure you are term active for the upcoming term.
• If a transfer student to NAU, submit proof of MMR to Campus Health Services.

Ongoing throughout program enrollment. The only acceptable CPR certification is American Heart Association BLS Provider.

Check out the different options you have to receive your CPR American Heart Association BLS Provider certificate:

1. Northern Arizona Healthcare for current American Heart Association CPR BLS Provider classes.
2. CPR Solutions for current American Heart Association CPR BLS Provider classes.
3. Visit the American Heart Association for other courses outside the Flagstaff area.

For Yuma Students:

1. Contact Onvida Health 928-336-1277.
2. Contact Arizona Western College 928-317-7674
3. Contact local fire stations (verify the course is AHA)
4. You may also visit the American Heart Association for other courses in your area.

You must have a current clearance card and it must be obtained through AZ DPS.

If you have a current Arizona State issued level 1 fingerprint clearance card, it will be accepted as long as it is not expired.

When completing the form, you will need to select Non-IVP and Heath and Sciences Student and Clinical Assistant.

If you have questions in regards to completing the application for your fingerprint clearance card, please contact Arizona DPS.

Once everything is completed you will need to make a PDF copy of your HIPAA completed training transcript and upload it to Typhon at nau.edu/mytraining.

Make sure your PDF copy shows that you completed all 7 units and the HIPAA University Security and Confidentiality Agreement. The College of Nursing does not need a copy of your HIPAA University Security and Confidentiality Agreement.

Legal Limitations for Nurse Licensure

For CBSN Summer 2026 – Liability insurance must be effective May 4, 2026, through May 4, 2027. Liability insurance can be purchased through NSO. The minimum amount required is $1 million each claim/ $6 million aggregate.

For TBSN and CBSN Fall 2026 – Liability insurance must be effective August 31, 2026, through August 31, 2027. Liability insurance can be purchased through NSO. The minimum amount required is $1 million each claim/ $6 million aggregate.

Required through program AND required to be renewed annually.

Nursing students are required to purchase magnetic name tags due to safety concerns associated with the use of safety pin name tags at certain clinical sites.

You can self-enroll in Bloodborne Pathogen training with this link https://ac.nau.edu/lms-apps/self-enroll/526979.

The page will then direct you to the home page within Canvas to launch the training. You will need to complete the modules and quiz to successfully finish the training.

Please upload a screenshot of your quiz grade (must be an 80% or above) to Typhon. At this time, there is no certificate of completion for the quiz.

If you are new to NAU, access to the Online Bloodborne Refresher Training requires enrollment in your nursing courses. Please check back after you have been enrolled in your nursing courses. Please contact University Admissions to ensure you have paid all fees and have been fully admitted to the university.

Having updated medical insurance is required throughout program enrollment.

You will need to submit a current copy of your personal major medical insurance card. If your name does not appear on the insurance card, please provide both the insurance card AND proof of dependent coverage.

NAU offers medical insurance that qualifies as full major medical coverage. If you are interested, please visit Campus Health for more information on the Student Health Insurance Plan (SHIP).

Physical Exam Form and Healthcare Provider Certificate

One physical exam required for entry into program.

Only the Physical Exam Form and Healthcare Provider Certificate that has been provided by the College of Nursing will be accepted as proof of completion. All other forms will not be accepted.

The forms need to be completely filled out and any incomplete documents will be rejected.

If you have been admitted to the College of Nursing on a provisional basis, please provide a copy of your unofficial transcript to the College of Nursing. Your provisional status needs to be cleared AT LEAST two weeks prior to the start of the term.

For students who are completing courses at NAU:

Northern Arizona’s College of Nursing Substance Use Policy and Procedure for Traditional, Compressed and Accelerated Pre-licensure Nursing Programs

Students in the College of Nursing are subject to all applicable substance and alcohol laws, regulations, and policies set forth by the Federal Government, State of Arizona, Board of Regents, Northern Arizona University, the College of Nursing and the American Nurses’ Association’s Code of Ethics. Students are subject to the substance use policies of the clinical settings to which they are assigned for clinical placement.

Substance screening and background check information will be sent out by each campus program coordinator with detailed information and due date. Final admission will be contingent upon results from drug testing and background check.

Substance screening and background check information will be required prior to final admission.

The substance screening must be conducted and turned in by date given. You will need to complete a background check AND a substance screening exam.

You will receive two emails from Certiphi to your NAU email address regarding your background check AND substance screening exam.

The cost of the substance screening test is $55.

The cost of the background check will be around $35 but can be higher if you have multiple aliases and addresses. Certiphi will place a $150 charge, but that is not typically what the cost will be.

For CBSN Summer 2026 – Uniforms must be ordered by March 2, 2026. Scrubin Uniform and Mission Linen are retailers who are authorized to use the NAU Sanghi College of Nursing logo. There are no other authorized retailers who are granted permission to use the NAU Sanghi College of Nursing logo, so students are unable to purchase personal scrubs and have the NAU logo sewn on.

For TBSN and CBSN Fall 2026 – Uniforms must be ordered by July 7, 2026. Scrubin Uniform and Mission Linen are retailers who are authorized to use the NAU Sanghi College of Nursing logo. There are no other authorized retailers who are granted permission to use the NAU Sanghi College of Nursing logo, so students are unable to purchase personal scrubs and have the NAU logo sewn on.

Hepatitis B

Proof of immunizations or proof of immunity by positive titer (must submit lab report for titer; immunization record needs to indicate 3-dose, 2-dose or 4-dose accelerated series).

• 3  dose series OR 2 Heplisav-B vaccinations required OR Positive Surface Antibody titer (lab report required)
• If titer is negative/low/equivocal, must repeat 3 dose or 2 dose Heplisav-B series
• If you are repeating the series please have your HCP indicate that on the document.
• Repeat titer is not required.
• Declination is accepted, however documentation from HCP stating the reason for declination is accepted.
• School form for declination is provided but not mandatory.

MMR

Proof of two immunizations or immunity by positive titer (must submit lab report for titer).

• 2 dose series required OR Positive Antibody titer for all 3 components (lab report required)
• If titer is negative/low/equivocal must repeat the 2 dose series OR BOOSTER dose as recommended by HCP.
• If you are repeating the series please have your HCP indicate that on the document
• Repeat titer is not required.

TB skin test or Quantiferon (blood test)

ALL students are required to complete the TB 2-step process. The first TB test is completed and then within 10 days, you must take a second TB test.

The 2-step TB skin test or blood test must be completed by the date given in Typhon.

Must be completed once a year and must be current throughout the program enrollment (positive TB skin test submit chest x-ray within 5 years or negative Quantiferon blood test).

• 2-Step TB Skin test (The first TB skin test is administered and then the 2nd TB skin step must be administered within 10 and 21 days) PPD skin test is accepted OR QuantiFERON Gold OR T-spot test is accepted (Lab report required).
• If 2 Step PPD or QuantiFERON Gold blood test is positive, provide a negative Chest X-Ray (valid for 5 years)

Tdap

Proof of one immunization within 10 years (TD nor DTAP are not accepted).

Varicella

• 1 or 2 dose series required OR Positive Antibody titer lab report required  (lab reports required)
• If titer is negative/low/equivocal must repeat the 2 dose series OR BOOSTER dose as recommended by HCP
• If you are repeating the series please have your HCP indicate that on the document
• History of disease is accepted but with positive antibody titer lab report

Influenza

Flu Immunization for the current flu season are required and must be for the current year, which is August to April. Annual influenza vaccines are due October 1st of every year, regardless if taken after October 1st of the previous year.

• Documented flu vaccination for current flu season OR completed and signed flu vaccine waiver of declination.
• Prescriptions/Receipts accepted only if it has administration date mention on it
• The flu vaccine should be available in August.
• Declination is accepted, however documentation from HCP stating a reason for declination is accepted.

Covid-19

Northern Arizona University and the College of Nursing do not require the Covid-19 vaccine; however, we will abide by the guidelines set by our clinical agencies.At this time, many clinical partners require all our faculty and students to have their Covid-19 vaccination completed before being allowed to attend clinicals. As a new admit student, you still have the choice to be unvaccinated. If you cannot attend clinicals due to being unvaccinated, there are no alternatives to acquire credit for clinical time.Please remember that attending clinicals is part of the curriculum in graduating with a Bachelor of Science in Nursing. If you are unable to attend clinicals, you will not receive a passing grade and be dismissed from the program. Please see the College of Nursing Student Handbook on receiving a failing grade.

• Pfizer Covid-19 vaccine – proof of two immunizations; or
• Moderna Covid-19 vaccine – proof of two immunizations; or
• Johnson & Johnson Covid-19 vaccine – proof of one immunization

After May 12, 2023:

• Bivalent Pfizer COVID-19 vaccine – proof of 1 immunization. OR

Bivalent Moderna COVID-19 vaccine – proof of 1 immunization.
