Revisions for Institutional Review Board approval


All research applications must be submitted to the Institutional Review Board (IRB) through IRBNet, an externally hosted software product. All IRB forms must be obtained from the IRBNet Forms and Templates library.

Conditions with approval must be submitted to the IRB to finalize approval for projects. This means a project with the status of Deferred–Modifications Required or Deferred–Information Required. The IRB must review and approve the modifications prior to conducting the research activities.

To submit a package for review you will need to create a new package within your currently approved project.

Items required for all revisions

  • Documents outlined in the IRBNet modifications letter. (E.g. informed consent form, project narrative, recruitment flyer, etc.) View the instructions for finding the IRBNet letter.
  • The PI must sign the package. (Tip: Student projects must be signed by the faculty advisor as well as by the student PI.)
  • Submit the package by clicking the “Submit” button in IRBNet. Select “Revision” from the “Submission Type” dropdown options.
