Religious Accommodation and Adjustment Request Form for Faculty, Staff, Affiliates, and Students
Consistent with our policies and practices, NAU will provide reasonable accommodation and adjustment for individuals to engage in university programs and activities based on their sincerely held religious beliefs and practices. Religion includes all aspects of religious observance, practice, and belief as well as moral or ethical beliefs as to what is right and wrong which are sincerely held. The Equity and Access Office oversees requests for religious accommodation and adjustment. To begin the interactive process seeking an accommodation or adjustment, please complete the applicable request form linked below. A member of the EAO team will reach back out to you through a trusted system, NAUGuardian.

Procedure for Student Academic related Religious Accommodation Requests
Students seeking to miss class or request other course-related adjustments to participate in a religious observance must submit the Religious Accommodation and Adjustment Request Form. The submission of the form initiates an interactive process that may include the student and relevant parties, as EAO deems necessary. If EAO approves the request, EAO will provide the student and faculty member with a memo detailing the approved adjustments.
Students must submit the request by the second week of a regular term course or as soon as possible upon learning of the need for the requested academic adjustment. In general, the university will not retroactively apply a religious adjustment.

Religion includes all aspects of religious observance, practice, and belief as well as moral or ethical beliefs as to what is right and wrong which are sincerely held. A sincerely held religious or moral or ethical belief does not have to be the majority belief or practice. It also does not have to relate to a “recognized” religion to be a sincerely held religious or moral or ethical belief.

Students must submit the request by the second week of a regular term/course or as soon as possible upon learning of the need for the requested academic adjustment. The Equity and Access Office (EAO) will review requests received after this time and notes that those received later in the term may be more challenging to implement in some circumstances.
