Making a Report to CIT

If you are aware of or have experienced behaviors that resulted in feeling unwelcome or excluded at Northern Arizona University, please complete the following form. Please be as specific as possible and include any relevant documentation such as photographs if applicable. The form allows for anonymous reporting, however, please understand that many incidents require follow up and without contact information there may not be much more than documentation of the incident that can occur.

We will respond to reports within 72 business hours.

The Campus Inclusion Team is a mandatory reporter under Title IX. Information related to gender and/or sex will be reported to the Office for the Resolution of Sexual Misconduct (ORSM).
