All IRB forms must be obtained from the IRBNet Forms and Templates library.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Investigators are responsible for concluding human research activities as soon as possible after a project is completed and\/or no longer involves human subjects.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Projects not renewed by the expiration date of the project are considered administratively closed by the Human Research Protection Program (HRPP) Office, and all human subjects research activities must cease as of the expiration date.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Closure submission<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">To close your project, you will need to create a&nbsp;<a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/New-Package-Tip.pdf\">new package<\/a>&nbsp;within your currently approved project.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Items required for all project closures<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Complete and upload the&nbsp;<a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/Closure-Tip.pdf\">Closure Form<\/a>.<\/li>\n\n\n\n<li>The <a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/Signing-Tip.pdf\">PI must sign the package<\/a>.
