All IRB forms must be obtained from the IRBNet Forms and Templates library.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Conditions with approval for acknowledgment means documentation is needed to be verified by the IRB for outlined research activities.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Response\/follow-up submission for approved with conditions<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Items required for all revisions<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Documents outlined in the IRBNet modifications letter. (E.g. informed consent form, project narrative, recruitment flyer, etc.) View the instructions for&nbsp;finding the&nbsp;<a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Decision-Letter.pdf\">IRBNet letter<\/a>.<\/li>\n\n\n\n<li>The&nbsp;<a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/Signing-Tip.pdf\">PI must sign the package<\/a>.
