All IRB forms must be obtained from the IRBNet forms and templates library.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Faculty members must serve as PI on undergraduate research applications.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-content-justification-center is-layout-flex wp-container-core-buttons-is-layout-fe48e5de wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button\"><a class=\"wp-block-button__link wp-element-button\" href=\"https:\/\/in.nau.edu\/research\/research-compliance\/human-research-protection-program\/irb-submission-process\/new-project\/student-application-instructions\/\" style=\"border-radius:30px\">Additional instructions for students<\/a><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Items required for all new projects<\/h2>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Complete the&nbsp;<a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/Application-Tip.pdf\">NAU research application<\/a>&nbsp;within IRBNet.<\/li>\n\n\n\n<li>Complete and upload the&nbsp;<a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/Project-Narrative-Tip.pdf\">Project narrative<\/a>.<\/li>\n\n\n\n<li>Complete and upload the&nbsp;<a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/Project-Personnel-Tip.pdf\">List of project personnel<\/a>.&nbsp;<strong>Tip:&nbsp;<\/strong>Ensure that all personnel on your project have completed human research&nbsp;<a href=\"https:\/\/about.citiprogram.org\/\" data-type=\"page\" data-id=\"2655\" target=\"_blank\" rel=\"noreferrer noopener\">Collaborative Institutional Training Initiative (CITI) training<\/a>, be sure to include yourself and your CITI training date.<\/li>\n\n\n\n<li>Upload a&nbsp;CV or biosketch for the PI, all CO-PIs, Co-Investigators, and the faculty advisor.<\/li>\n\n\n\n<li><a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/Sharing-Tip.pdf\">Share your project&nbsp;<\/a>with the NAU IRB Office, granting \u201cFull\u201d access.<\/li>\n\n\n\n<li>The <a href=\"https:\/\/in.nau.edu\/research\/wp-content\/uploads\/sites\/349\/2026\/06\/Signing-Tip.pdf\">PI must sign the package<\/a>. Student projects must be signed by the faculty advisor as well as by the student PI.<\/li>\n\n\n\n<li>Submit the package by clicking the \u201cSubmit\u201d button in IRBNet.&nbsp;Select \u201cNew Project\u201d from the \u201cSubmission Type\u201d dropdown options.<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\">Additional requirements<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Based on the type of research you are doing you may also need to submit the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Informed consent\/permission\/assent form(s)\u2013including study-specific release of information documents, DHHS approved sample consent forms.
