If you are currently or have been an employee (including student workers) within the last calendar year, you MUST contact Human Resources<\/a> to make changes to your Primary Name.<\/p>\n
If you HAVE NOT<\/span> been an employee within the last calendar year, you must complete the Change of Name Form<\/a>, attach a copy of your birth certificate, a driver’s license, a passport, visa, social security card, marriage license, divorce decree, a court order for a legal name change, or US tax ID card showing your legal name and submit it to the Office of the Registrar<\/a>.<\/p>\n<\/body><\/html>\n\n <\/div>\n<\/div>\n\n

Change of Preferred Name\n Accordion Closed<\/span><\/h4>\n <\/span>\n <\/div>\n <\/a>\n

Current students can indicate a “preferred” (or chosen) first name, which is a name the student wishes to be commonly known as, if different from the student’s legal first name. As long as the use of the preferred first and\/or middle name is not for the purpose of misrepresentation, it will appear instead of the person’s legal name on Class Rosters, Grade Rosters, Course Management System (Canvas), and the Online Directory. The student’s legal name will remain unchanged in all other university related systems where the use of the legal name is required by university business or legal need.<\/p>\n
Please complete the Change of Name Form<\/a>, with your Preferred name and submit the form to the Office of the Registrar<\/a>.<\/p>\n<\/body><\/html>\n\n <\/div>\n<\/div>\n\n

Your Degree Name must be your legal name or a derivative of your legal name.<\/p>\n

Current students may update their Preferred Pronouns and Gender Identity<\/a> by logging into LOUIE; clicking on the “Profile” tile; clicking on “Personal Details” from the left hand menu; clicking on the “Biographic” tab; and then select an option from the drop down menus for Preferred Pronouns and\/or Gender Identity.<\/p>\n

Please use the Change of Information Form<\/a> to update Date of Birth and\/or Legal Sex<\/a>.<\/p>\n
To make changes, you must attach a copy of your birth certificate, a driver’s license, a passport, or visa showing your correct date of birth and\/or you must attach a copy of your birth certificate, a driver’s license, a passport, a visa, a letter from a health care provider, or legal paperwork showing your correct sex and submit the form to the Office of the Registrar<\/a>.<\/p>\n
