• NAU PD Anonymous Report
• Discrimination/Harassment Report must checkmark the “Remain Anonymous” box in the “Background Information” Section
• Title IX Sexual Misconduct Report must checkmark the “Remain Anonymous” box in the “Background Information” Section
• Conflict of Interest Consultation Report must checkmark the “Remain Anonymous” box in the “Background Information” Section
• Report a concern: NAU social Media Site. The contact information on the site form is optional. You may submit a social media concern anonymously
• Making a report to Campus Inclusion Team (CIT) The form allows for anonymous reporting; however, many incidents require follow-up and without contact information there may not be much more than documentation of the incident that can occur
