You should also respond to inquiries from Human Resources and your department in a timely manner. You are required to provide Human Resources and your department up to date contact information (e-mail, phone and home address) while applying for a leave of absence and for the duration of your leave of absence if approved.<\/p>\n<h2>Report your time away from work<\/h2>\n<p>Until your leave of absence is evaluated, you are required to report your time away from work through your timesheet.\u00a0You should to use the regular time reporting codes in your timesheet. For example, Vacation, Sick and Leave Without Pay. If your Leave of Absence is approved you will have access to special \u2018Leave of Absence&#8217; (LOA) time reporting codes.\u00a0 For example, LOA &#8211; Vacation, LOA &#8211; Sick and LOA &#8211; Leave Without Pay.<\/p>\n<h2>Accommodation<\/h2>\n<p>If you have a physical or mental impairment that substantially limits your ability to perform the essential functions of your job, you may qualify for reasonable accommodations.
