Unless taking a planned vacation, employees should request a LOA if they need to spend five or more consecutive days away from work.<\/p>\n<p><em>If not on an approved leave of absence time away from work could be considered an unauthorized absence.<\/em><\/p>\n\n<\/div>\n<!-- shortcode-column -->\n<div class=\"shortcode-column shortcode-column--count-2\">\n    \n<p><img decoding=\"async\" class=\"alignnone wp-image-17867 size-landscape-image\" src=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2023\/06\/Teddybear-temp-photo-by-kristine-wook-on-unsplash-1-464x348.jpg\" alt=\"Image of taking a teddy bear's temperature \" width=\"464\" height=\"348\" srcset=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2023\/06\/Teddybear-temp-photo-by-kristine-wook-on-unsplash-1-464x348.jpg 464w, https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2023\/06\/Teddybear-temp-photo-by-kristine-wook-on-unsplash-1-800x600.jpg 800w, https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2023\/06\/Teddybear-temp-photo-by-kristine-wook-on-unsplash-1-232x174.jpg 232w\" sizes=\"(max-width: 464px) 100vw, 464px\" \/><\/p>\n\n<\/div>\n<\/div>\n<hr role=\"separator\" class=\"hr--transparent hr--transparent-15px not-in-view\">\n<h2><strong>Reasons for a LOA<\/strong><\/h2>\n<p>Eligible employees may request a LOA for reasons including:<\/p>\n<div class='shortcode-column-container'><!-- shortcode-column -->\n<div class=\"shortcode-column shortcode-column--count-2\">\n    \n<ul>\n<li>the employee\u2019s health condition<\/li>\n<li>to care for a spouse, parent, or dependent child<\/li>\n<li>birth or bonding with a child<\/li>\n<\/ul>\n\n<\/div>\n<!-- shortcode-column -->\n<div class=\"shortcode-column shortcode-column--count-2\">\n    \n<ul>\n<li>military service and training<\/li>\n<li>bereavement<\/li>\n<li>bone marrow or organ donation<\/li>\n<li>personal reasons including education<\/li>\n<\/ul>\n\n<\/div>\n<\/div>\n<hr role=\"separator\" class=\"hr--transparent hr--transparent-15px not-in-view\">\n<h2><strong>Family Medical Leave <\/strong><\/h2>\n<p>Family Medical Leave (FMLA) is available to eligible employees who may need to take time away from work due to personal illness, family illness or other qualified family matters.\u00a0 An eligible employee may request a total of twelve weeks of FMLA during any 12-month period.<\/p>\n<p><em>You may be eligible to request a FMLA leave if you:<\/em><\/p>\n<ul>\n<li>have worked for the university for a continuous period of not less than one year, and<\/li>\n<li>have worked at least 1,250 hours in the prior 12-month period.<\/li>\n<\/ul>\n<p><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2024\/02\/Leave_Your-Rights-and-Responsibilities-Family-Medical-Leave.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Review Your FMLA Rights &amp; Responsibilities.<\/a><\/p>\n<hr role=\"separator\" class=\"hr--transparent hr--transparent-15px not-in-view\">\n<h2><strong>How to request a LOA<\/strong><\/h2>\n<p>You request a leave of absence by completing the online\u00a0<a href=\"https:\/\/onbase.nau.edu\/appnet\/UnityForm.aspx?d1=AU1DZYqd67du9wn8qcUsqi40qz3etbz4eD0%2BYSUAbQH4xyh8e607c%2BteqpQ2q%2FjneGRJHwm%2BeiY5dho8ZpBRje2F9fqoe16nq5wo77L%2FM7wwxM2Y04Mklb8ats1MZ47f3dToDJ7M5c9klDvJN84u5K7e49jwrTE8twkkuYKHjF7FpCVOuL00Ah4NgBFoB6ng4pawnDcoqxGS0Gb0iD2XFpUbsiPM6LUUT4wCjONSHai%2B\">Leave of Absence Request form<\/a>.\u00a0Human Resources will evaluate your request and notify you if you are eligible for a FMLA or approved time off under another type of leave of absence.<\/p>\n<p style=\"text-align: left;\"><a href=\"https:\/\/onbase.nau.edu\/appnet\/UnityForm.aspx?d1=AU1DZYqd67du9wn8qcUsqi40qz3etbz4eD0%2BYSUAbQH4xyh8e607c%2BteqpQ2q%2FjneGRJHwm%2BeiY5dho8ZpBRje2F9fqoe16nq5wo77L%2FM7wwxM2Y04Mklb8ats1MZ47f3dToDJ7M5c9klDvJN84u5K7e49jwrTE8twkkuYKHjF7FpCVOuL00Ah4NgBFoB6ng4pawnDcoqxGS0Gb0iD2XFpUbsiPM6LUUT4wCjONSHai%2B\" target=\"_blank\" rel=\"noopener noreferrer\"><\/a><\/p>\n<hr role=\"separator\" class=\"hr--transparent hr--transparent-15px not-in-view\">\n<div class=\"eplus-bg-color-row bg-color-dark-blue\"><div class=\"eplus-bg-color-row-content\">\n<p><strong>Rules to remember<\/strong><\/p>\n<ul>\n<li>Whenever possible you should request a leave of absence at least\u00a0<strong>30 days <\/strong>in advance.<\/li>\n<li>If the LOA is unplanned, you should follow your department\u2019s call-in procedure.<\/li>\n<li>You should talk to your supervisor about your LOA request, so your department can plan for your time away from work.<\/li>\n<li>Be sure to provide your supervisor and Human Resources your contact information.<\/li>\n<li>You need response to inquiries from your supervisor and Human Resources promptly.<\/li>\n<li>Human Resources will communicate to you through your work email. If you need to use another type of communication, please inform your leave advisor.\u00a0 <\/div><\/div><\/li>\n<\/ul>\n<hr role=\"separator\" class=\"hr--transparent hr--transparent-15px not-in-view\">\n<h3><strong>Income replacement<\/strong><\/h3>\n<p>Most LOAs provide <strong>unpaid<\/strong> time away from work, but there are several ways for an employee to replace some or all of their normal income while on a LOA.<\/p>\n<ul>\n<li><strong>Paid Time Off:<\/strong> Sick, vacation or compensatory time.<\/li>\n<li><strong>Disability Payments:<\/strong> Short term and long-term disability.<\/li>\n<li><strong>Workers Compensation<\/strong>: Worker\u2019s compensation benefits.<\/li>\n<li><strong>Compassionate Transfer of Leave:<\/strong> Donated vacation time. <a href=\"https:\/\/nau.edu\/university-policy-library\/hr_4-09\/\">Review policy for details<\/a><\/li>\n<\/ul>\n<hr role=\"separator\" class=\"hr--transparent hr--transparent-15px not-in-view\">\n<h2>Reporting time away from work<\/h2>\n<p>All time way from work should be reported on your timesheet in Employee Self-Service through <a href=\"https:\/\/hr.peoplesoft.nau.edu\" target=\"_blank\" rel=\"noopener\"><strong>LOUIE &#8211;
