or reserve a conference room when they need to work on-site.<\/li>\n<li>Individuals working 3 to 4 days remote, or are working the majority of their standard work week remote, will need to work with their supervisor on shared or &ldquo;swing-space&rdquo; Agreement<\/h4>\n<p>Have an agreed upon Flexible Workplace Options Request between the employee and supervisor.<\/p>\n<h4>Step 2. Request form<\/h4>\n<p>The employee or supervisor can submit the request using the Flexible Workplace Options Request (OnBase form).<\/p>\n<ul>\n<li>Within the form complete the required fields. Please do not include any personal or medical information in the notes section.<\/li>\n<li>Include work schedule details such as work days, hours, or any flexible work arrangements in the provided text box.<\/li>\n<li>By clicking submit as the employee you approve the request and it moves forward to your supervisor.\n<ul>\n<li>The supervisor reviews to approve or deny.<\/li>\n<li>If approved the request moves to the next level supervisor up to the Associate Vice President and\/or Dean level for final approval.<\/li>\n<li>If denied, the request is not longer active and a new request is needed.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><em>Having issues with the form try &ndash; <a href=\"https:\/\/servicenow.nau.edu\/sp?id=kb_article&amp;article=KB0013129\" target=\"_blank\" rel=\"noopener\">Clear Browser Cache<\/a><\/em><\/p>\n<h4>Step 3. Evaluate and communicate<\/h4>\n<p>Identified regular work schedules should be evaluated often to address any needed changes. A schedule change that is consistent and on-going will require a new Flexible Workplace Options Request.<\/p>\n<hr>\n<h4 style=\"text-align: center\">When and how often to submit a new Flexible Workplace Options Request?<\/h4>\n<ul>\n<li><strong>At the beginning of the new fiscal year (July 1st)<\/strong><\/li>\n<li><strong>When an employee has a change in supervisor<\/strong><\/li>\n<li><strong>When an employee has a long term and consistent change to their work schedule<\/strong>\n<ul>\n<li>Temporary and short term changes can be discussed and agreed
