There are three important things to consider when selecting your medical, dental and vision coverage.<\/p>\n

Cost: <\/strong>Assess the costs you expect in the coming year. Employee Premiums + Out-of-Pocket Costs: Deductibles, Co-pays & Coinsurance.<\/li>\n
Network: <\/strong>To determine if your provider is in the plan’s network you go the plan’s website<\/a> and do a provider search or call customer service<\/a> for the vendor.<\/li>\n
Benefits and Coverage: <\/strong>Review the Benefit Plan Documents and Policies<\/a> for the coverage details for each plan.<\/li>\n
The Summary of Benefits and Coverage<\/a> provides coverage details for medical plan in easy to understand language.<\/li>\n

Visit the Vendor Contact webpage<\/a> which includes member ID number, group number, the provider’s website address and customer service phone number. <\/strong><\/p>\n<\/body><\/html>\n\n <\/div>\n<\/div>\n\n

Your enrollment information will be sent to the vendor on the date your benefits are effective.<\/p>\n

It takes several days before your enrollment is shown on the provider’s eligibility portal.<\/li>\n
Once on the portal, you can register for an account with the vendor and print a temporary ID Card.<\/li>\n

Your benefits are effective the first of the pay period following your elections, however it may take up to 2 weeks for the vendors to receive and process your information. Eligible NAU HDHP plan <\/strong>members can access Campus Health Services but are subject to the plan’s deductible and coinsurance.<\/li>\n

With this plan you want to make sure that your provider is in the network. There are no out-of-network benefits, and the network is limited, especially in Northern Arizona.<\/p>\n
