These regulations require group health plans and health insurance issuers that offer coverage to provide access to the SBC and Uniform Glossary.<\/p>\n<p>You these documents can be found on <a href=\"https:\/\/in.nau.edu\/Human-Resources\/Benefits\/Benefit-Plan-Document\/\" target=\"_blank\" rel=\"noopener noreferrer\">Plan Documents<\/a> web page under the Summary of Benefits and Coverage tab<\/p>\n<h3><strong>Consolidated Omnibus Budget Reconciliation Act (COBRA)\u00a0 | <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2022\/02\/NOTICE-NAU-GENERAL-COBRA.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">General Notice<\/a><\/strong><\/h3>\n<p>Notice of the Arizona Benefit Options Program COBRA Coverage. This notice has important information about your right to COBRA continuation coverage, which is a temporary extension of coverage under the Plan.<\/p>\n<h3>Health Insurance Portability &amp; Accountability Act (HIPAA) | <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2022\/02\/NOTICE-NAU-HIPAA-PRIVACY.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Notice<\/a><\/h3>\n<p>This notice protects the privacy of individually identifiable health information and establishes who can use the personal health information and how it can be used.<\/p>\n<h3><strong>HIPAA Special Enrollment Rights | <\/strong><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2022\/02\/NOTICE-NAU-HIPAA-SPECIAL-ENROLLMENT-RIGHTS.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Notice<\/a><\/h3>\n<p>This notice provides information regarding special enrollment. You may be able to enroll in health plan without waiting for the next open enrollment period if you request a QLE change within 31 days of an event for loss of other coverage or birth, adoption or placement for adoption. Also, you may enroll if you become eligible for a state premium assistance program under Medicaid or CHIP. You must request enrollment within 60 days after you gain eligibility for medical plan coverage.<\/p>\n<h3>Medicare Notice of Creditable Coverage | <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/5\/2022\/02\/NOTICE-NAU-CREDITABLE-COVERAGE.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Notice<\/a><\/h3>\n<p>This notice has information about the prescription drug coverage through the Benefit Options program for participants with Medicare.
