Update 11/12/2021: The Safer Federal Workforce employee vaccination requirement due date has been extended from December 8, 2021 to January 18, 2022 to be fully vaccinated. Employees are encouraged to continue to check back on this page for continuing updates.

General Vaccine Requirement Questions
Who is required to get vaccinated? (Updated 11/17/2021) Accordion Closed
All employees of NAU are required to comply with the federal contracting requirements. This means that administrators, faculty, and staff (including student employees, graduate assistants, and postdoctoral scholars) must all be vaccinated; part-time temporary employees must also be vaccinated. This also includes new hires and employees rehired after a break in service.
For purposes of the requirement, ‘employees’ includes anyone who receives a paycheck from NAU. Therefore, non-paid interns, volunteers, affiliates, and vendors are not included in this requirement.
When do I need to be fully vaccinated? Accordion Closed
- Employees must be fully vaccinated and must upload proof of vaccination by January 18, 2022.
What if I believe I am eligible for an accommodation or exemption? Accordion Closed
If I’ve already had COVID-19, am I still required to get vaccinated? (New 11/17/2021) Accordion Closed
I’ve already had COVID-19 and my doctor told me to wait to get vaccinated. How can I meet the requirement? (New 11/17/2021) Accordion Closed
If your doctor has advised a waiting period that will not allow you to meet the January 18, 2022 deadline, you will need to request a medical accommodation through Disability Resources at firstname.lastname@example.org.
Will I be paid for the time needed to get the vaccination for COVID-19? (New 11/17/2021) Accordion Closed
Yes. All employees will be allowed to use a reasonable amount of their paid work time in order get a vaccine(s).
Information for Supervisors and Hiring Officials
Should supervisors be confirming that employees have uploaded their vaccine information? (New 11/17/2021) Accordion Closed
No. Supervisors do not need to, nor should they be, asking for or managing employee vaccine information.
Do new hires need to comply with the vaccine requirement? (New 11/17/2021) Accordion Closed
Yes. NAU expects all new hires to be vaccinated unless they have an approved medical or religious accommodation. Proof of vaccination will be required by hire date (or by January 18, 2022).
Will a compliance statement be added to offer letters and job postings? (New 11/17/2021) Accordion Closed
Yes. The following language is being added to all offer letter templates and job postings:
Under the recent executive order issued by President Biden requiring employees of federal contractors to receive COVID-19 vaccinations, NAU expects all employees, including new hires, to be vaccinated unless they have an approved medical or religious accommodation. Should we talk about the vaccine requirement with candidates? (New 11/17/2021) Accordion Closed
You may only inform candidates of the vaccination requirement and accommodation request options by directing them to the FAQs for Employee Vaccination Requirement. Applicants or finalists must not be asked if they are vaccinated during the hiring process.
Can I offer a revised start date if an applicant would like more time to fulfill the vaccine requirement? (New 11/17/2021) Accordion Closed
Yes. Hiring officials have the discretion to choose a hire date that works for all parties. Hiring officials should make sure any delayed hire dates meet their business needs.
I was vaccinated at NAU Campus Health Services; do I need to upload my card? Accordion Closed
No, if you were vaccinated at Campus Health Services your vaccination record will already be in the system. There is nothing further for you to submit.
If I was vaccinated at the NAU Fieldhouse, do I still have to upload my card? (New 11/17/2021) Accordion Closed
Yes. The NAU Fieldhouse site is a state vaccination site and not tied directly to Campus Health Services. Because of this, you will still need to upload your card through the Campus Health Patient Portal.
I’ve already been vaccinated, but not at NAU Campus Health Services. How do I upload my card? Accordion Closed
- Log into the Campus Health Patient Portal and choose “NAU Student/Employee” and login with your NAU credentials. Click “Enter my COVID-19 Vaccine Information” and follow the on-screen prompts.

How can I check if my card has been uploaded? Accordion Closed

I lost my vaccination card. Where can I get a replacement? Accordion Closed
- To request a replacement card, contact the vaccination provider site where you received your vaccine.
- If that site is no longer operating, submit a record request to the Arizona Department of Health Services (ADHS) using the ADHS Immunization Record Request Form. This can take 5-7 days or possibly longer.
- To potentially access your records faster, ADHS recommends using MyIR Mobile (a free service) to find your record.
I received my vaccine in another country. Will it be accepted here? (New 11/17/2021) Accordion Closed
COVID-19 vaccines currently approved or authorized by the U.S. Food and Drug Administration (Pfizer-BioNTech, Moderna, and Johnson & Johnson [J&J]/Janssen COVID-19 vaccines) or COVID-19 vaccines that have been listed by the World Health Organization (e.g., AstraZeneca/Oxford) will be accepted. Proof of vaccination must be uploaded to the Campus Health Patient Portal.
