High Deductible Health Plan & Health Savings Account (HSA)This option is a combination of two types of plans with the network through Aetna. Network websites: Aetna Review the in-network benefit High Deductible Plan This plan has a high deductible which you will need to meet before most benefits are paid. The plan uses the Aetna Choice POS II network. For out of network services, members are responsible for any balance billing amounts. Members must first meet all deductibles. The family deductible is tracked as a whole—not individually Health Savings Account Under this plan, pre-tax contributions are made by both you and NAU into an account from which you can pay for out-of-pocket expenses.
- Employer: NAU contributes $27.70 per pay period for employee only coverage and $55.39 for all other coverage levels.
- Employee: You can elect to contribute your own contributions to your HSA account up to the IRS annual maximum. Please note your contributions AND NAU's contributions, together cannot exceed the IRS annual limit.

Dental Health Maintenance Organization (DHMO)Network website: www.cigna.com/sites/stateofaz/ Review the in-network benefit The State of Arizona offers a DHMO through CIGNA Dental. To receive any benefit under a dental DHMO, you need to use providers included in the Plan's 'Exclusive’ network of providers. Non-emergency services received from providers not the Plan's 'Exclusive' network are not covered. A limited number of dentists in the CIGNA network , in particular in Northern Arizona. States where CIGNA Dental is not offered: Alaska, Hawaii, Maine, Montana, New Hampshire, New Mexico, North Dakota, Puerto Rico, Rhode Island, South Dakota, Vermont, West Virginia and Wyoming. You should not elect CIGNA Dental unless you are willing to use one of the network dentists. You can find out which dentists are in the network by calling CIGNA customer service at 800.968.7366 or you can go to https://www.cigna.com/sites/stateofaz/.

Discount Program Accordion Closed
This program will provide each member with substantial discounts on vision exams and corrective materials received from provider that are in the network. There are no out-of-network benefits. Providers not participating in the program will not honor any of the discounted fees. The member will be responsible for full retail payment. Review of offered discounts If you do not enroll in the Avesis Advantage option you will be automatically enrolled in the Avesis Discount Program at no cost to you.

Health Accordion Open
Health Care FSAThis account reimburses you for eligible out-of-pocket medical and related health care expenses such as copays, coinsurance or prescriptions for you or any of your eligible dependents during the plan year.
For 2019 you can contribute up to $2,650 annually to the Health Care FSA. You may use your full annual election prior to having the full amount of payroll contributions deposited to your account.
Find out how your contributions will be affected if you are placed on an approved leave of absence.
- At the end of the plan year the health care FSA offers you the ability to rollover $500 to use in the next plan year.
- Funds will only rollover for individuals who are actively working on the last day of the plan year.
- John elects to contribute $2,000 to the Health Care Flexible Spending Account for the 2017 calendar year.
- Between January 1, 2017 and December 31, 2017 John incurs $1,400 of eligible claims.
- John’s balance as of December 31, 2017 is $600.
- $500 would be ‘rolled over’ to the next plan year and $100 would be forfeited.

- Eye glasses, contacts, contact solution and LASIK surgery
Example ineligible expenses:
- Premiums for medical and dental insurance
- Over the counter medications without a prescription
- Long term care expenses
FSA Dependent Care Accordion Closed
This account reimburses you for any child and dependent day care expenses that would otherwise be eligible for a tax credit by the IRS. For example: Day Care or a private sitter. Eligible expenses can only be reimbursed up to the current balance of contribution in your account. Before you incur an expense, it is a good idea to determine if it is eligible for reimbursement on the ASI website, asiflex.com. Note: Dependent medical and/or other expenses should be submitted the the Medical FSA not the Dependent Care FSA.

Important: The Dependent day care FSA do not have the $500 rollover provision.
Changing your dependent care elections
You may be able to change your dependent day care election outside of open enrollment if you experience a change in your dependent care eligibility.
Dependent day care FSA vs. Tax CreditA Dependent day care FSA may not provide the greatest tax advantage for all employees – greater tax savings may be gained by claiming the available tax credit. Consult with your tax advisor to determine which option is best for you.
- Services provided by a licensed day care facility for six or more children
- Babysitting services while you are at work
- Private school tuition including kindergarten
- Babysitting while you are not at work

You are automatically covered for $15,000 of basic life insurance provided by The Hartford at no cost to you.
- Non-smokers will receive an additional $1,000 benefit.
- $15,000 of Accidental Death and Dismemberment (AD&D) insurance
- $15,000 Seat Belt Benefit may also be payable if you die in an automobile accident.
- The benefit will be paid to your designated beneficiary (ies).
Supplemental Life Insurance Accordion Closed
Coverage amount: You may purchase coverage of one, two, or three times your salary (rounded to the nearest $1,000) up to a maximum of $300,000. Newly eligible: You may purchase up to the maximum amount. Open enrollment: If enrolled, you may elect an additional one times your annual salary.
Dependent Life Insurance Accordion Closed
Coverage amount: You may purchase coverage in increments of $5,000 up to thew lesser of, three times your salary, or $300,000. Newly eligible: You may purchase up to the maximum amount. Open enrollment: You may increase, decrease or stop coverage. Elections or changes are in multiples of $5,000 up to a $20,000 maximum annually.
Short Term Disability Insurance Accordion Closed
You may purchase Short Term Disability (STD) through either The Hartford or Unum Provident for income replacement coverage for a non-work related accident or illness. There is a maximum benefit period of 26 weeks. Contributions are after tax so benefits are tax-free. When evaluating the cost of Short Term Disability coverage you need to look at the amount of income replacement provided for the employee premium paid. Note: The benefit amount, waiting period and premium of these two plans are very different.
- The employee premium for Hartford is lower but so is the amount of income replacement the Hartford provides.
- The employee premium for Unum is higher but so is the amount of income replacement Unum provides.
Long Term Disability Insurance Accordion Closed
Long term disability insurance protects you from loss of income in the event you become disabled for more than six months as a result of illness or non-work related injury. You are required to participate in a long-term disability plan and you will be automatically enrolled into the long-term disability plan that corresponds to your mandatory retirement plan.
- Arizona State Retirement System (ASRS) - If you are participating in ASRS, you receive your long-term disability benefits from Broadspire.
- Optional Retirement Plan: If you are participating in the Optional Retirement Plan (ORP), the Public Safety Personnel Retirement System (PSPRS), or if you are a Post-Doctoral Scholar you are covered by The Hartford Insurance Company
