All IRB forms must be obtained from the IRBNet Forms and Templates library.<\/p>\n<p>Application to request that IRB oversight be deferred to a non-Northern Arizona University IRB.<\/p>\n<h2>Ceded review submission<\/h2>\n<p><strong>All ceded reviews require the following:<\/strong><\/p>\n<ol>\n<li>Complete and upload the <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Ceded-Review-Tip.pdf\">Ceded Review form<\/a>.<\/li>\n<li>Complete and upload the <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Project-Personnel-Tip.pdf\">List of Project Personnel<\/a>\u00a0listing NAU personnel only.<\/li>\n<li>Approval letter from the reviewing Institution.<\/li>\n<li>Original approved application from the reviewing Institution.<\/li>\n<li>Original approved informed consent from the reviewing Institution.<\/li>\n<li><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Sharing-Tip.pdf\">Share your project<\/a> with the NAU IRB Office, granting \u201cFull\u201d access.<\/li>\n<li><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Signing-Tip.pdf\">The PI must sign the package.<\/a>.
