All IRB forms must be obtained from the IRBNet Forms and Templates library.<\/p>\n<p>Conditions with approval for acknowledgment means documentation is needed to be verified by the IRB for outlined research activities.<\/p>\n<h2>Response\/follow-up submission for approved with conditions<\/h2>\n<p>To submit a package for acknowledgment you will need to create a <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/New-Package-Tip.pdf\">new package<\/a>\u00a0within your currently approved project.<\/p>\n<p><strong>All submissions require the following:<\/strong><\/p>\n<ul>\n<li>Documents outlined in the IRBNet determination letter. (E.g. \u00a0site authorization, translated informed consent form(s), etc.) View the instructions for <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Decision-Letter.pdf\">finding the IRBNet letter<\/a>.<\/li>\n<li><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Signing-Tip.pdf\">The PI must sign the package.<\/a>\u00a0(Tip: Student projects must be signed by the faculty advisor as well as by the student PI.)<\/li>\n<li>Submit the package by clicking the \u201cSubmit\u201d button in IRBNet.\u00a0Select \u201cResponse\/Follow-up\u201d from the \u201cSubmission Type\u201d dropdown options.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Response\/follow-up All research applications must be submitted to the IRB through\u00a0IRBNet, an externally-hosted software product.
