All IRB forms must be obtained from the IRBNet Forms and Templates library.<\/p>\n<p>Conditions with approval must be submitted to the IRB to finalize approval for projects. This means a project with the status of Deferred \u2013 Modifications Required or Deferred \u2013 Information Required. The IRB must review and approve the modifications prior to conducting the research activities.<\/p>\n<h2>Revisions submission (modifications required and\/or information required)<\/h2>\n<p>To submit a package for review you will need to create a <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/New-Package-Tip.pdf\">new package<\/a> within your currently approved project.<\/p>\n<p><strong>All revisions require the following:<\/strong><\/p>\n<ul>\n<li>Documents outlined in the IRBNet modifications letter. (E.g. informed consent form, project narrative, recruitment flyer, etc.) View the instructions for <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Decision-Letter.pdf\">finding the IRBNet letter<\/a>.<\/li>\n<li><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Signing-Tip.pdf\">The PI must sign the package.<\/a>.
