All IRB forms must be obtained from the IRBNet forms and templates library.<\/p>\n<p><a href=\"https:\/\/in.nau.edu\/human-research-protection-program\/irbnet\/new-project\/instructions-for-student-applications\/\">Instructions for student applications<\/a><\/p>\n<ul>\n<li>Faculty members must serve as PI on undergraduate research applications.<\/li>\n<\/ul>\n<h2>All new projects require the following:<\/h2>\n<ol>\n<li>Complete the <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Application-Tip.pdf\">NAU research application<\/a>\u00a0within IRBNet.<\/li>\n<li>Complete and upload the <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Project-Narrative-Tip.pdf\">Project narrative<\/a>.<\/li>\n<li>Complete and upload the <a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Project-Personnel-Tip.pdf\">List of project personnel<\/a>. <strong>Tip: <\/strong>Ensure that all personnel on your project have completed human research <a href=\"https:\/\/in.nau.edu\/human-research-protection-program\/irbnet\/new-project\/training\/\">Collaborative Institutional Training Initiative (CITI) training<\/a>, be sure to include yourself and your CITI training date.<\/li>\n<li>Upload a\u00a0CV or biosketch for the PI, all CO-PIs, Co-Investigators, and the faculty advisor.<\/li>\n<li><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Sharing-Tip.pdf\">Share your project<\/a> with the NAU IRB Office, granting \u201cFull\u201d access.<\/li>\n<li><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/255\/2023\/01\/Signing-Tip.pdf\">The PI must sign the package.<\/a>. Student projects must be signed by the faculty advisor as well as by the student PI.<\/li>\n<li>Submit the package by clicking the &#8220;Submit&#8221; button in IRBNet.\u00a0Select \u201cNew Project\u201d from the \u201cSubmission Type\u201d dropdown options.<\/li>\n<\/ol>\n<h2>Additional requirements:<\/h2>\n<p>Based on the type of research you are doing you may also need to submit the following:<\/p>\n<ul>\n<li>Informed consent\/permission\/assent form(s) &#8211; including study-specific release of information documents, DHHS approved sample consent forms. If consent will not be documented in writing, a script of information to be provided orally to subjects.<\/li>\n<li>Data collection tools &#8211; surveys, questionnaires, data elements for records reviews, etc.<\/li>\n<li>Participant materials &#8211; written materials to be provided to or meant to be seen or heard by subjects (e.g. instructional materials, study newsletters, physician to participant letter, wallet cards, incentive items, holiday\/birthday cards, certificates, etc.)<\/li>\n<li>Recruitment materials &#8211; telephone and verbal scripts, flyers, classroom announcements, brochures, websites, email texts, radio\/television spots, newspaper advertisements, press releases, etc.<\/li>\n<li>Grant application(s) and notice(s) of grant award &#8211; cover-to-cover copy of grant and award letter, regardless of home institution or funding agency.<\/li>\n<li>Site authorizations for research purposes and\/or access to administrative records\/samples.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>New IRB project All research applications must be submitted to the IRB through\u00a0IRBNet, an externally-hosted software product.
