We welcome these visitors and make their safety our top priority.<\/p>\n<p>Before a minor is eligible to participate in projects and activities at Northern Arizona University, the project supervisor or principal investigator (PI) or her\/his designee must make sure the following forms are reviewed and submitted to the\u00a0<a href=\"mailto:nauehs@nau.edu\">Office of Environmental Health and Safety<\/a>.<\/p>\n<p>The supervisor or PI or her\/his designee must review and submit:<\/p>\n<ul>\n<li><a href=\"https:\/\/nau.edu\/university-policy-library\/research\/\">Minors Policy<\/a><\/li>\n<li><a href=\"https:\/\/in.nau.edu\/wp-content\/uploads\/sites\/226\/2020\/03\/Minors-Final-Potential-Hazards-Form-12.pdf\">Potential Hazards Form for Minors<\/a><\/li>\n<\/ul>\n<p>The minor&#8217;s parent\/legal guardian must review the following form and submit it to the Insurance Officer at NAU Property and Liability Insurance Services, PO Box 4067.<\/p>\n<ul>\n<li><a href=\"https:\/\/nau.edu\/wp-content\/uploads\/sites\/26\/Assumption-of-Risk-Waiver-Release-and-Consent-for-Non-Student-Minor-Participation.pdf\">Parental\/Legal Guardian Consent Form<\/a><\/li>\n<\/ul>\n<p>Pre-planning is key to this process as an EHS site visit will be required prior to project initiation.
