|Summer Camp Registration Form||Download this form and complete fully before submitting to the Rec Center Front Desk. Please complete in black or blue ink!|
|Information Card||Download and complete this document along with your Registration Form. Registration cannot be completed without it.|
|Climbing Wall and Challenge Course Waiver||Download and complete this document along with your Registration Form and Information Card. Registration without this waiver will result in your child not be able to participate in Climbing Wall and/or Challenge Course activities with their group.|
|Payroll Deduction Form||NAU Faculty/Staff are eligible to use Payroll Deduction if they register for all of Session I and Session II.|
|Lemon Memorial Scholarship Application||This scholarship provides financial assistance for Flagstaff Public School students who have a demonstrated financial need and who wish to participate in NAU Campus Recreation group swim lessons or Summer Youth Program. Payment for camp must be made in full as this scholarship is awarded on reimbursement basis only.|
|Alumni Verification||This form is the verification for any Alumni who has not already gotten a membership at Campus Recreation. Please read through thoroughly.|
Registration is limited. The Explorers Camp is limited to 20 campers each week. The Mountain Jacks Camp is limited to a total of 95 campers each week. Register as soon as possible to reserve your space
Important registration information
Accepted forms of payment
- Credit Card
- Money Order
- Cashier’s Check (Personal Checks NOT accepted)
- Payroll Deduction (NAU Faculty/Staff ONLY)
- You must register in person at the Campus Recreation Front Desk. Payment is due at registration.
- Bring completed forms to the Rec Center Front Desk between the hours of 8:00am – 5:30pm, Monday – Friday with full payment.
Same day registrations will not be accepted. Please be prepared for the following:
- Payments made Friday of the week prior to attending camp = begin camp the following Tuesday.
- The deadline to register for a week of camp is Monday of that week. Payment during mid-week will not be accepted.
Need to make a change or request a refund?
All purchases from NAU Campus Recreation (including memberships, program registrations, and all other services) are non-refundable and non-transferable. Exceptions to this policy may include requests made for medical or institution transfer reasons, but all refunds are subject to a 20% administrative fee. To request a refund, please email email@example.com. To register for additional weeks from your initial order, please complete a second Registration form and submit in-person.
Childcare Voucher Program
If you are a student parent, you may be eligible to receive an award to help cover the cost of registration.
