Upload your insurance card<\/h1>\n

In order to ensure that you are charged the correct rate, please make sure that Campus Health has the most up to date insurance card. Insurance cards must be reuploaded January and when you begin attending or working at NAU.<\/p>\n

Insurance cards must be uploaded within 24 hours of your CHS appointment.<\/strong><\/p>\n

Mobile upload instructions<\/h3>\n

  1. Sign in to your patient portal account<\/a> as an NAU Student<\/li>\n
  2. Click the menu icon in the upper-left corner<\/li>\n
  3. Select “Insurance Details”<\/li>\n
  4. Select “Add New Card”<\/li>\n
  5. Scroll to the bottom of the window to select “Upload Front Image”<\/li>\n
  6. Take a picture of your card using your phone camera (landscape\/horizontal orientation is required)<\/li>\n<\/ol>\n

    2. Select “Upload Back Image” and repeat process for back of card<\/li>\n<\/ol>\n
