Telemedicine lowers risk of exposure while maintaining accessibility to your healthcare provider. Telemedicine visits are billable services but all insurance plans may not cover telemedicine, even in these changing times.<\/p>\n<p>We advise all patients who wish to utilize CHS telemedicine for medical or psychiatric care to reach out to their insurance company. Ask whether Telemedicine services are a covered benefit under your plan. Telemedicine services will be billed at the same cost as an in-office, In person visit.<\/p>\n<hr \/>\n<p>Our services are available for all currently-enrolled Northern Arizona University students who have paid the mandatory semester Health and Wellness Fee. Insurance is not required to receive our services; however, it is suggested.\u00a0 Services are also available for Northern Arizona University staff and faculty and their adult dependents.<\/p>\n<h3><strong>Plans<\/strong><\/h3>\n<p>You can choose from these four plan options in order to get treatment from Medical Services:<\/p>\n<ul>\n<li><a href=\"https:\/\/in.nau.edu\/campus-health-services\/fee-for-service\/\"><span style=\"text-decoration: underline\">Fee for Service<\/span><\/a>: You pay for individual services and are charged an office visit along with any other labs or procedures you have done.<\/li>\n<li><a href=\"https:\/\/in.nau.edu\/campus-health-services\/louiecare\/\"><span style=\"text-decoration: underline\">LouieCare<\/span>:<\/a> You pay $175 per semester and most services provided by Medical Services are covered with a $15 co-pay per service.\n<ul>\n<li>The <a href=\"https:\/\/in.nau.edu\/campus-health-services\/louiecare\/\">LouieCare<\/a> must be paid in full to be in effect. Any services received before the plan is paid in full will not be covered at the discounted rate.<\/li>\n<\/ul>\n<\/li>\n<li><a href=\"https:\/\/in.nau.edu\/campus-health-services\/major-medical-insurance\/\">Student Health Insurance Plan (SHIP):<\/a> This group health insurance product is offered by all three state universities in Arizona and provides group coverage to students who are uninsured, need a less expensive alternative, or need an Arizona provider network. Primary care is covered at Medical Services with a $15 co-pay for each office visit and $10 for any lab.<\/li>\n<li><strong>Your own contracted insurance<\/strong>: <span data-teams=\"true\">We are currently contracted with PPO plans with Aetna, Blue Cross Blue Shield, Cigna, TRICARE, and United Healthcare. We are not contracted with any HMO plans, state Medicaid plans (AHCCCS, Medi-Cal, etc.) or any other insurances not previously listed.\u00a0<\/span><\/li>\n<\/ul>\n<h3>Additional information:<\/h3>\n<ul>\n<li>CHS must receive insurance information (copy of front and back of card) in order to bill any contracted insurance.\u00a0 You may upload your card images prior to your appointment through the patient portal:\u00a0 <a href=\"https:\/\/www.campushealth.nau.edu\">www.campushealth.nau.edu<\/a>.\u00a0 The card may also be uploaded at your appointment. \u00a0If the insurance information is received after the date of service, any incurred charges may become the patient\u2019s responsibility.<\/li>\n<li><span data-teams=\"true\">Complete the <strong>Insurance Details<\/strong> section and read and acknowledge the <strong>Billing Form<\/strong> in the Patient Portal <strong>before your visit to CHS.<\/strong><\/span><\/li>\n<li><span data-teams=\"true\">Any copays, deductibles, or denied charges will be posted to your LOUIE account. By signing the consent to treat, you authorize us to release information to your insurance company to process your claim. Recognize, however, if the billing address for your insurance plan is your parent&rsquo;s\/spouse&rsquo;s\/significant other&rsquo;s home, an EOB from the insurance company (see insurance How Insurance Works) will be sent to that address.
