Learn about the circumstances that could qualify, and the documentation required for a policy exception request.<\/p>\n<p>The ASC will not consider appeals without supporting documentation. You must submit your documents by the\u00a0<a title=\"deadline.\" href=\"http:\/\/in.nau.edu\/academic-standards-committee\/student-procedures\/\"><u>deadline<\/u>.<\/a><\/p>\n<h2>Medical issues<\/h2>\n<p>Medical issues that may qualify include:<\/p>\n<ul>\n<li>physical or psychological difficulty<\/li>\n<li>family medical difficulty<\/li>\n<\/ul>\n<h3>Required documentation for medical issues<\/h3>\n<p>To request a policy exception for medical reasons, you must provide a signed letter on official letterhead from the attending health care provider, counselor, or licensed practitioner. The letter must specify the following:<\/p>\n<ul>\n<li>the date of onset of illness or accident<\/li>\n<li>the dates you or your family member was under professional care<\/li>\n<li>the general nature of the medical condition<\/li>\n<li>how it prevented you from following normal university policies and procedures<\/li>\n<\/ul>\n<p>The following are not sufficient documentation:<\/p>\n<ul>\n<li>file notes<\/li>\n<li>medical receipts<\/li>\n<li>copies of prescriptions<\/li>\n<\/ul>\n<h3>Personal issues<\/h3>\n<p>Personal issues that may qualify include:<\/p>\n<ul>\n<li>a death of a family member<\/li>\n<li>a legal issue<\/li>\n<li>an accident<\/li>\n<li>the loss of employment due to forced layoff or military deployment<\/li>\n<\/ul>\n<h3>Required documentation for death of family member<\/h3>\n<p>You need to provide documentation that verifies the date of death within the term for which you are requesting a policy exception. Acceptable documentation includes a copy of the:<\/p>\n<ul>\n<li>death certificate<\/li>\n<li>obituary<\/li>\n<li>memorial service program<\/li>\n<\/ul>\n<h3>Required documentation for other personal issues<\/h3>\n<p>A signed letter on official letterhead from an appropriate official such as:<\/p>\n<ul>\n<li>an attorney<\/li>\n<li>a law enforcement agent<\/li>\n<li>a court official<\/li>\n<li>another appropriate source<\/li>\n<\/ul>\n<p>The signed letter needs to specify:<\/p>\n<ul>\n<li>the date of the incident<\/li>\n<li>the dates you or your family member was affected by the incident<\/li>\n<li>the general nature of the incident<\/li>\n<li>how it prevented you from completing your course work and following normal university policies and procedures<\/li>\n<\/ul>\n<p>Note: The dates in the official letter from the appropriate official must be congruent with the circumstances and timelines cited in the exception request.<\/p>\n<p>If your petition is related to the impact of gender or sexual based violence (Sexual Assault, Domestic\/Dating Violence, Stalking, Sexual Harassment, etc.) please contact the Office of Student Life at 928-523-5181 prior to submitting this form.<\/p>\n<h2>Situations that do not qualify for a policy exception<\/h2>\n<p>You are expected to accept responsibility for your academic performance unless there have been extenuating circumstances as described above.<\/p>\n<p>The following circumstances are not considered situations that merit an exception:<\/p>\n<ul>\n<li>academic difficulty, such as failing grades and\/or desire to clean-up your academic record<\/li>\n<li>a change in major<\/li>\n<li>a change in career goals<\/li>\n<li>overcommitted in areas such as course load, student activities, and employment, without additional circumstances beyond your control<\/li>\n<\/ul>\n<h2>How to send in documentation<\/h2>\n<h3>E-mail<\/h3>\n<p>Send supporting documents to\u00a0<a href=\"mailto:AcademicStandards@nau.edu\">AcademicStandards@nau.edu<\/a>.<\/p>\n<h3>Drop off<\/h3>\n<p>Supporting documentation that cannot be sent electronically should be brought to Peterson Hall, Building 22, Room 323.<\/p>\n<h3>Postal mail<\/h3>\n<p>You can also mail documents to:<\/p>\n<p>Academic Standards Committee<br \/>\nPO Box 4091<br \/>\nFlagstaff, AZ 86011<\/p>\n<h3>Fax<\/h3>\n<p>Fax documents to <a href=\"tel:928-523-1922\">928-523-1922<\/a>.
